Advance Care Planning and Residential Goals of Care in Aged Care | Online

Overview:
There is an increasing emphasis from government agencies and healthcare providers to improve the completion rates of Advance Care Planning (ACP) documents. This webinar will highlight the challenges of balancing community expectations with the right of individuals to make autonomous decisions regarding their health and personal care.

This education will clarify the role of staff in embedding ACP into practice, ensuring it benefits both residents and staff in supporting the provision of person-centred care. It will also highlight how Residential Goals of Care (RGoC) and an Advance Health Directive (AHD) can be used in conjunction to guide decision-making.

Learning Outcomes:

2. Recognise the role of each Advanced Care Planning (ACP) document.
