DESCRIPTION:We all want to have a say in what happens in the last part of our lives. By making plans that cover our care\, lifestyle\, health and finances\, we work out what we want\, as well as making things easier on those around us. This process is called Advance Care Planning. \nThis free online workshop will cover information on what is involved in Advance Care Planning\, how to get started\, and how to talk with your loved ones about your end-of-life wishes. It is a practical and interactive workshop where you will have the chance to get your questions answered. \nThis workshop will be presented by Sabena Lund who runs Palliative Care WA’s community education program. \n  \nYou must register for this free online workshop here on Eventbrite. After you register we will: \n\nPost you a resource pack that you need to participate in the workshop\, and\nCloser to the time\, email you the Zoom link to use to access the workshop.\n\nIf you register by 16 June we will post you the resource pack. If you live in regional or remote areas of WA we recommend you registering as soon as possible to give extra time for the resource pack to reach you. Anyone registering after then\, will receive their pack via email.

So like any other significant life event\, it’s well worth your consideration and a little forward planning. \nThis process is called advance care planning. Advance care planning is the process of discussing and documenting your future health care wishes\, values and priorities. It enables your family\, friends\, carers and health professionals to understand what you want\, should you become so unwell that you are unable to speak for yourself. More than 80% of Australians think it’s important to put our end-of-life care preferences into writing\, but less than 5% of us have completed an advance care plan. \nBOOKINGS ARE ESSENTIAL as places are limited.
