DESCRIPTION:Wednesday 6 May 2020\nRegistration 7am\n7.15-8.15am \nDesigned for\nGeneral Practitioners and Practice Nurses in a primary health setting \nOverview\nGoals of Care (GoC) is now more relevant than ever before as we deal with the biggest health crisis in our lifetimes. \nJoin Dr Jacquie Garton-Smith\, alongside Ms Val Colgan\, to discuss where GoC fits in care planning and end-of-life conversations\, and its relevance for GPs in practice. \nThis session will enable you to get a greater understanding of the use of GoC\, the My Health Record\, HealthPathways WA and other resources.
