An exciting opportunity to develop your skills by working with the Indigenous people of the Pilbara awaits. The role of the Care Coordinator involves working collaboratively with GeneralPractices, AMSs, and other service providers within the Pilbara to assist Aboriginal and Torres Strait Islander eligible clients with chronic disease care coordination.
As this role will include clinical care, a qualified nurse or Aboriginal Health Worker qualifications are required to fill this position. If you have good communication skills, an understanding of the chronic disease, and the ability to work well within a team focusing on chronic disease, we would like to hear from you.
A valid current driver’s license “C” class is a requirement of this position.
Current working with children’s check (or willing to obtain)
Current police clearance (or willing to obtain)
Current CPR certificate
Current AHPRA registration
Base Salary $ 71,750.00 plus allowances, plus superannuation
